Job description
Optum Home & Community Care is seeking experienced Senior Community Care clinicians to deliver integrated care in nursing homes, senior housing, and home settings. You will perform comprehensive assessments, coordinate advanced illness planning, and partner with the care team to improve patient outcomes.
Qualifications include ANCC/AANP or NCCPA credentials, active state licensure, and DEA prescriptive authority where required.
