Job description
Florida Department of Health in Duval County seeks a Senior Community Health Nurse to lead nursing care in a community health center and satellite sites. You will coordinate case management for patients with chronic diseases and HIV, administer vaccines and medications, and ensure accurate documentation in the EHR.
