Senior Counselor of OnTrack

FLSA Status: Exempt

Salary Range: $48,000 - $56,000. Personalized salary reflecting your related experience and academic/credentialed background.

Community Differential for this program: Earn up to $5,000 annually

Location: Buffalo, NY 14201

The Senior Counselor works at a maximum level of independence and responsibility, often sharing responsibility for program operations with the Program Director. The majority of time is spent in direct client services, providing crisis and ongoing counseling and psychotherapy to new and ongoing clients.

OnTrack (430 Niagara Street, Buffalo, NY) - OnTrack is a Coordinated Specialty Care (CSC) treatment program serving adolescents and young adults who are within two years of experiencing the onset of psychosis. Our mission is to provide individualized care to keep young people on track with their goals for school, work and personal relationships. The team has the opportunity for creative and flexible engagement in the office and in the community, establishing collaborative relationships and creating new resources to support young people and families. Equity, inclusion, rapid access and self-determination are at the core of everything we do. Looking for a full time licensed Counselor.

RESPONSIBILITIES:

Provide clinical supervision to assigned staff. May supervise student interns

Reviews and signs treatment plans developed by program staff.

Assigns new cases to staff according to client needs and staff skills.

Conducts utilization review of client charts to determine completeness of chart, appropriateness of treatment, and medical necessity for continued care or recommendations for alternate care.

Provides initial and ongoing assessments of clients, including client history, evaluation of problems, and formulation of diagnosis.

Develops initial and ongoing treatment plans which describe the goals of treatment and the methods to be used.

Provides individual, family and group counseling and psychotherapy to assist clients in achieving the goals of treatment.

Assists clients with linkages to appropriate community resources to enhance treatment and recovery.

With appropriate releases of information, communicates with others in client service systems to coordinate care. Such contract may include, but not limited to, primary care physicians, insurance companies and other service providers involved in the care of clients.

Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

QUALIFICATIONS: Master's Degree and Qualified Health Professional (QHP) certification (LMSW, LCSW, LMHC) required.

Must have knowledge of a variety of common intervention techniques, common mental health diagnoses, substance use interventions and resources.

Excellent communication skills.

Must be familiar with community resources to be able to appropriately link clients.

Minimum proficiency with Windows based software applications is required.

Must have experience with Electronic Medical Records databases and basic computer skills.

Must appreciate cultural diversity and be aware of cross cultural counseling issues.

Valid Drivers License, vehicle in working condition (with valid inspection, registration, and insurance), ability to travel between sites, & Defensive Driving Course (only applicable for some of our community & mobile programs).
