Job description
HeartShare is hiring a Sr Day Support Professional to ensure the implementation of person-centered services for each individual and to maintain their physical and emotional well-being. You will develop and review staff action plans, coordinate monthly schedules for community outings, and train staff on plans and documentation.
This is a full-time day shift in Brooklyn, NY with travel up to 25%. Initial compensation starts at an hourly base of $24.50 with potential bonuses and raises based on
