Job description
ChenMed is seeking a Primary Care Physician (PCP) to join a physician-led, value-based organization delivering VIP care to seniors. The PCP will manage a panel of up to 450 patients, coach patients toward health goals, and lead a care team to ensure high-quality, cost-effective outcomes.
Responsibilities include independent patient management, collaboration with hospitalists, care coordination, and participation in risk and quality programs.
