Job description
Hcr Home Care is seeking a Full Time RN Case Manager in New York to provide skilled nursing care at patients' homes in Oswego County and the Rochester area. You will coordinate care with physicians, families and the nursing team, ensuring evidence-based practice and timely documentation.
The role requires an active NYS RN license, a minimum of one year in home care, and strong communication, critical thinking, and organizational skills.
