Job description
Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to provide primary care in skilled nursing and senior living facilities. You will deliver direct care, promote wellness, manage chronic conditions, and coordinate with other providers.
You’ll work in SNFs, assisted living, and independent living communities, with a focus on reducing hospital readmissions and improving quality of life for residents.
