Job description

Twin Cities Physicians in Minnesota is seeking a Physician (MD/DO) for Senior Living Rounding and Mobile Primary Care across community-based settings. You’ll lead care for medically complex seniors, develop treatment plans, and collaborate with NPs, PAs, nurses, and care teams to improve outcomes.

The role emphasizes proactive, relationship-based medicine, reducing hospitalizations, and delivering high-quality care within senior living communities.
