Job description
UnitedHealth Group seeks highly qualified Advanced Practice Clinicians (NPs or PAs) to join the Senior Community Care team providing care to residents in long-term care, senior housing, and home care settings. Expect comprehensive assessments, sub-acute rehabilitation support, and collaborative care planning with an interdisciplinary team.
Requires active NP/PA certification, state licensure, and DEA where applicable; strong EMR skills and reliable transportation are essential.
