Job description
Optum Home & Community Care is seeking a Senior Advanced Practice Clinician (NP/PA) to join the SCC program, coordinating care for high-risk members in long-term care, senior housing, and home settings. You will conduct assessments, manage complex needs, and work with an interdisciplinary team to improve patient outcomes.
The role requires active NP or PA certification, unrestricted licensure in the practicing state, and DEA licensure eligibility.
