Job description
George Mark Children’s House seeks a Lead RN Case Manager to coordinate pediatric palliative care under the Director of Clinical Programs. This full-time exempt role requires about 40 hours weekly and occasional local travel for hospitals and events.
Responsibilities include coordinating admissions, transfers, and discharges, chart reviews, care planning, and EMR updates to support families. The leader will mentor a case team and ensure HIPAA compliance.
