Job description
The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to join our rehabilitation team. You will assess referred patients, develop and administer individualized treatment plans, document patient progress and educate families on therapy goals.
Requirements include a valid state license, supervision of Physical Therapy Assistants, and strong documentation skills; experience in long-term or geriatric care is preferred.
