Job description
The Chatham Hills Subacute Care Center is actively seeking an experienced Physical Therapist to assess and treat patients in a subacute facility setting in New Jersey. You will establish and carry out treatment plans, guide PT Assistants, and document patient progress to support recovery and functional goals.
Ideal candidates will have a PT degree and current state licensure, with experience in long-term or geriatric care and a strong focus on accurate documentation and interdisciplinary
