Job description
The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) to join our facility in a geriatric rehabilitation setting. The PT will assess referred patients, develop and administer treatment programs, and establish goals with physician directions.
Knowledge of documentation and interdisciplinary collaboration is essential. Responsibilities include supervising PT assistants, maintaining accurate progress notes, and educating patients and families about therapy plans.
