Job description
CenterWell Senior Primary Care, part of Humana, is seeking a Nurse Practitioner or Physician Assistant to deliver comprehensive senior care onsite at the State Ave. clinic in Kansas City, MO.
You will collaborate with a multidisciplinary team to provide value-based care, coordinate referrals, and support hospital/SNF transitions while documenting care with coders. A state license and TB screening are required, and Spanish language skills are preferred.
