CHOICE of NY in White Plains, NY is seeking a Senior Program Director to supervise the Health Home Care Management Program and oversee the Criminal Justice Health Home component.
You will manage staff, ensure compliance with Health Home regulations, develop cross-department collaborations, and represent the program at external meetings.
This full-time, hybrid role offers a 35-hour work week with a compensation range of $70,000 to $80,000 annually.
