Job description
UC San Diego Health is seeking an RN Care Manager to facilitate patient care from pre-admission through discharge in a tertiary acute hospital. You will coordinate with physicians, nurses, social workers, and other providers to expedite appropriate, cost-effective care and discharge planning.
Key qualifications include California RN licensure, BART or BLS, and at least 3 years of inpatient RN care management experience with strong organizational and communication skills.
