Job description
21st Century Home Health Services seeks a Registered Nurse, Level 2 Home Health Case Manager to lead a small team of two nurses. You will assess patients in the field, develop and implement care plans, and coordinate with physicians to deliver high-quality home health care in Sebastopol and surrounding areas.
The role requires CA RN licensure, current BLS/CPR, and strong leadership and interdisciplinary collaboration skills.
