Job description
HCR Health is seeking a dedicated RN Case Manager to administer skilled nursing services for patients in Cobleskill and surrounding areas. You will teach families, coordinate care, and ensure services align with physician orders.
You will handle admissions, assess patients, develop and supervise care plans, oversee Home Health Aides, document thoroughly, and meet regulatory requirements while maintaining high patient outcomes.
