Job description
Matter of Care, a CenterLight Health System member, is seeking a Clinical Assessment Manager (RN) – Intake to lead initial assessments for PACE participants in Queens and surrounding areas.
You will perform in-home and virtual assessments using UAS-NY, evaluate eligibility, coordinate with intake and care teams, educate families about PACE benefits, and ensure CMS/NYSDOH compliance with meticulous EMR documentation.
