Senior Social Worker - Patient Aligned Care Team (PACT)
The Senior Social Worker - Patient Aligned Care Team (PACT) is assigned to serve community-based, outpatient clinics across all of North Florida and South Georgia. This position is located at Ocala, FL. The Senior Social Worker is primary responsible for providing psychosocial and mental health assessment, diagnosis, treatment planning/execution, care coordination, case management, crisis intervention, and evidence-based psychotherapy service to Veterans and their family members/caregivers.
