Job description
Mount Sinai Health System is seeking a Senior Social Worker for Palliative Medicine and Geriatrics at Union Square / Downtown. The role involves psychosocial screening, discharge planning, and coordination of complex care, with direct patient/family counseling and community referrals.
The successful candidate will manage a clinically complex caseload, provide supervision to staff/interns, and participate in interdisciplinary rounds and program development to improve patient care and outcomes.
