Job description
CenterWell Home Health seeks a Speech-Language Pathologist to provide in-home evaluation and treatment for seniors and adults with chronic conditions. In this role, you’ll assess speech, language, cognition, and swallowing, develop individualized care plans, and deliver evidence-based therapy to improve safety, communication, and independence at home. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using electronic systems.
