Day Shift: 5x8-Hour (08:00 AM - 05:00 PM)
Position Overview
This role involves providing skilled home care services across multiple areas, including Manatee County and South Hillsborough. The successful candidate will be responsible for the management and rehabilitation of patients ranging from adolescents to geriatrics, addressing acute and chronic conditions.

Certifications required: BLS, CCC-SLP, and state license.
One year of relevant experience is required.
Must possess a valid driver's license and vehicle for home visits.
COVID-19 vaccination is required, with exemptions considered.
Flu vaccination or declination is necessary; mandatory mask-wearing while at work.
Job Responsibilities
The position requires delivering home skilled visits, with a minimum of 6 points per day as per manager's discretion. The expected number of visits ranges from 6 to 8 per day. Candidates should be flexible and willing to cover multiple areas as needed.

Additional Information
Hours and visits will be self-scheduled based on the needs of the home care office. Candidates must understand the need for flexibility concerning the scheduling and areas covered.
Patient Demographics
Responsible for managing patients across various age groups, specifically from adolescence to geriatrics. The candidate will work with patients who have both acute and chronic conditions.
Interdisciplinary Support
This position collaborates with various support services including Physical Therapy, Respiratory Services, Social Services, and Pharmacy.
Equipment and Special Procedures
There may be special equipment required for certain patient interactions and assessments. Flexibility and adaptability to various patient needs are essential.
