Job description
Creative Health Solutions in Fort Wayne, Indiana is seeking a skilled Speech Therapist to join our therapist-led team at Skilled Nursing facilities. You will develop and administer therapy programs to improve patients' physical, mental, and emotional well-being.
Collaborate with a multidisciplinary team to create personalized treatment plans, document progress, and communicate outcomes to physicians and care staff. A valid state license is required, and ongoing CEUs are available.
