nSUMMARY OF JOB
nSocial Services coordinator position identifies the needs of each resident/patient/families and assures that the needs are met by the appropriate discipline. Assists patients/residents/families in improving /maintaining and their ability to manage their physical, mental and psychosocial needs.

nRECRUITMENT REQUIREMENTS
nnnBSW or BA in sociology/social work or related human services/healthcare field
n
nnMinimum one year experience in a long-term care setting or equivalent related healthcare experience
n
nnExcellent oral and written communication skills
n
nnKnowledge of community resources
n
nnExperience working as part of an interdisciplinary team
n
nnComputer experience
n
