Albany Home Health & Hospice seeks an experienced Start of Care Registered Nurse (SOC RN) to join our clinical team in Albany, OR. This full-time role emphasizes patient assessment, care planning, and coordination for home health and hospice patients.

You will perform SOC visits, complete OASIS assessments, develop plans of care, coordinate with the interdisciplinary team, educate patients and caregivers, and ensure timely documentation and regulatory compliance.
