Social Rehabilitation AssistantMt Lebanon Blvd - Pittsburgh, PA 15234OverviewPosition Type Full Time Job Shift M-F Day Shift Education Level High School or GEDDescriptionChartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. We take pride in being innovative in helping those we serve achieve personal goals by creating opportunities to align our services and supports with those goals and to help them in their recovery.As a Social Rehabilitation Assistant with Chartiers Center the core job duties include:Assess consumer needs related to social rehabilitation using standardized assessment tools, direct observation, and individualized evaluation techniques.Design, coordinate, and facilitate therapeutic interventions, activities, and events that address identified consumer needs, with an emphasis on group-based programming.Conduct required assessments and evaluations utilizing program-specific and standardized tools to monitor consumer progress and outcomes.Collaborate in the development and implementation of individualized therapeutic and rehabilitative plans based on assessed consumer needs.Foster a positive, supportive therapeutic environment by serving as a role model, encouraging social interaction, and providing consistent support and reinforcement.Maintain accurate, timely, and complete documentation in accordance with program, agency, and regulatory requirements.At Chartiers Center we offer the following benefits:Competitive SalaryFull-Time Monday-Friday with flexibility of hoursMedical, Dental and VisionShort- and Long-Term DisabilityLife InsuranceRetirement Plan (Employer Contribution 6% after one year)8 Paid HolidaysGenerous PTO (Paid Time Off)Tuition ReimbursementCompany car to be used for company businessFree ParkingMinimum Qualifications:Associate's degree (or equivalent) with one-year behavioral health experienceOR High School Diploma/GED and two years behavioral health experienceValid Driver's License RequiredDiversity and Inclusion- Each of our clients/individuals is uniquely different and so are we.
