Westwood Specialty Care , a 85-bed long-term care skilled nursing community located in Sioux City, IA, is now hiring!

nAs a Social Services Coordinator on our team, you will have the opportunity to apply your skills and demonstrate your compassion, positively influencing the lives of our residents. Together, we can make a difference in the lives of our residents and their loved ones.
nWhat You'll Do & Key Responsibilities:
nnnAssess residents’ psychosocial needs and complete social histories
n
nnComplete assigned MDS sections accurately and on time
n
nnDocument care plans, progress, and follow-up per regulations
n
nnProvide counseling and emotional support to residents and families
n
nnSupport residents and families with adjustment, rights, and orientation
n
nnAssist with discharge planning and care transitions
n
nnCoordinate referrals and act as a liaison with community resources
n
nnPartner with the care team to enhance quality of life and outcomes
n
nnPlan and support resident, family, and community engagement activities
n
nnPromote facility programs and services to referral partners
n
n nQualifications:
nnnA bachelor’s degree in social work, gerontology, or a related field within health and human services including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.
n
nnAbility to obtain a Licensed Social Worker (LSW).
n
nnMinimum of one year of supervised social work experience in a health care setting working directly with individuals.
n
nnInterpersonal and communication skills that identify the ability to positively and effectively communicate with others while completing work accurately and timely.
n
nnA strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.
