Job Summary:
Lake Placid Health and Rehabilitation Center, a trusted member of the AOM/Eagle Point Healthcare family, is seeking a compassionate and experienced Social Services Director to join our team in Lake Placid, Florida. As a key member of our leadership team, you will oversee the social services department, ensuring our nursing home's commitment to individualized care and quality of life for our residents. Responsibilities:
Oversee the social services department, including case management, social work, and recreational therapy services
Develop, implement, and manage programs to meet the social, emotional, and spiritual needs of residents
Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans
Recruit, train, and supervise social services staff to ensure exceptional service delivery
Conduct assessments and support groups for residents and their families
Develop and maintain relationships with community organizations and healthcare providers
Participate in quality improvement initiatives to enhance the overall resident experience
Ensure compliance with regulatory requirements and industry standards
Requirements:
Bachelor's degree in Social Work, Counseling, or a related field (or equivalent experience)
Minimum 1-2 years of experience in social services, preferably in a long-term care setting
Strong leadership, communication, and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks
Familiarity with Florida's Long-Term Care Ombudsman program and elder law
