Social Services Director — Lead Care & Community Liaison

Job description

Allaire Health Services, LLC seeks a Social Services Director to lead resident social care programs within our facility. You will participate as a team member addressing MDS sections and care plans, maintain social service records, and supervise assessments for new and current residents.

This role also covers discharge planning, acting as a liaison between the facility and the community, and reporting on residents' status with accurate documentation.
