Social Services Director Skilled Nursing
Job Summary:
We are seeking a compassionate and experienced Social Services Director Skilled Nursing to join our team. As a critical member of our care team, you will be responsible for ensuring the highest level of emotional, social, and psychological well-being for our residents.
Key Responsibilities:
Develop and implement comprehensive social services plans to meet the unique needs of each resident
Conduct comprehensive assessments and provide individualized care plans to promote resident independence and autonomy
Coordinate and facilitate resident activities, including life-enrichment programs, outings, and social events
Collaborate with interdisciplinary care teams to ensure holistic care and address resident concerns and needs
Provide emotional support and counseling to residents, families, and caregivers
Develop and maintain relationships with community organizations and service providers to enhance resident services
Ensure compliance with state and federal regulations regarding social services
Supervise and direct social services staff, including social workers, activity directors, and other support staff
Requirements:
Minimum of (insert minimum years of experience) years of experience in social services or a related field
Bachelor's degree in Social Work, Psychology, or a related field (or equivalent experience)
Strong communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced, dynamic environment
Experience with electronic medical records and software
Valid driver's license and reliable transportation)
