Social Services MSW
Join our team as a Social Services MSW and make a meaningful impact in patient care at Providence Saint John's Health Center.
What You'll Be Doing
Provide case management services to patients
Collaborate with healthcare teams to develop care plans
Conduct assessments and evaluations of patient needs
Facilitate communication between patients and healthcare providers
Support patients and families through the healthcare process
What We Need To See
Master's degree in Social Work (MSW)
At least 3 years of experience in social work
Experience with Epic software
Availability to work every other weekend
