Job description
Home Health/Hospice Medical Social Worker (Msw)
The MSW will serve in several developmental roles inclusive of providing professional medical social services for both hospice and home health. Additional responsibilities will include a bereavement program. It is anticipated that as the agency grows, and additional staff positions added, that the predominant work responsibility will be medical social services primarily to hospice and home health patients and their families.
Essential Job Function:
Provide living opportunities within nurturing environments of hope, dignity and love.
Assess the psychosocial status of patients and families.
Carry out social evaluations and plans interventions or support services to patients and family/caregivers. Makes referrals as appropriate.
Assist with maintaining a bereavement program.
Maintain clinical records on all patients served.
Maintain collaborative relationships with organizational personnel and outside agencies.
Participate in discharge planning as appropriate.

Essential Job Function: Employee Responsibilities
Follows policy and procedures on TB testing.
Follows policy and procedures on annual influenza vaccinations.
Attends all mandatory in-services
Works under the general supervision of the Home Health & Hospice Administrator
Has daily contact with the potential patients and their families
Has daily contact with nursing home direct care staff to communicate pertinent patient admission information
Requirements:
Minimum of two years experience in health care, home health / hospice experience preferred.
Education Requirement: Meet or exceed CMS/Medicare regulatory requirements for home health and hospice.
MSW from accredited school.
Physical Demands: Requires minimal lifting functions Requires moderate bending, standing and reaching activities.
Working Conditions: Works in client's homes and in office areas. Driving and travel required. This position requires the ability to function independently in performing assigned duties.
