Location: Chicago, IL
Facility: Ascension Saint Joseph
Department: Case Management
Schedule: Day shift | PRN
How you'll make an impact in this role
Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values (LSW required, LCSW preferred).
Monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services.
Advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care.
Maintain ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.
What minimum requirements you'll need
Licensure / Certification / Registration:
One or more of the following required:
Clinical Counselor credentialed from the Illinois Department of Financial and ProfessionalRegulation obtained prior to hire date or job transfer date.
Counselor credentialed from the Illinois Department of Financial and Professional Regulationobtained prior to hire date or job transfer date.
Social Worker specializing in Clinical Practice credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences:
Previous in patient discharge experience needed
