Job description
Licensed Clinical Social Worker (LCSW)
A dynamic opportunity for a Licensed Clinical Social Worker (LCSW) is available. The ideal candidate will possess a Master's Degree and have at least 2 years of relevant experience, including travel experience preferred. This position involves a commitment to providing high-quality care and support to patients and their families.
Schedule
Work Hours: 8 hours/day, Monday to Friday, from 8:00 AM to 4:30 PM
Holiday Requirement: Available as needed
Weekend Requirement: Not required
Key Responsibilities
Conduct comprehensive psychosocial assessments and develop, implement, and adjust individualized treatment and discharge plans.
Manage a full caseload of patients with moderate to high complexity, including those with co-occurring disorders and psychosocial instability.
Provide crisis intervention and evidence-based therapeutic services across individual, group, and family settings.
Lead discharge planning efforts, identifying and resolving barriers related to housing, insurance, legal status, and access to care.
Coordinate complex transitions of care, ensuring continuity across inpatient, outpatient, and community-based services.
Ensure compliance with regulatory, legal, and accreditation standards, including patient rights, mandated reporting, and documentation requirements.
Manage court coordination and legal processes with accuracy and timeliness.
Maintain high-quality documentation reflecting clinical decision-making and patient progress.
Qualifications
Master's Degree required.
License and Certification: LCSW, Epic experience, BLS, CPI.
Minimum of 2 years of experience required; travel experience preferred.
New graduates will not be considered for this position.
