Job Overview
Our client is seeking a licensed Social Worker to join a multidisciplinary team at an onsite facility in Trenton, NJ. The ideal candidate will support patients and their families through case management, treatment planning participation, and ongoing clinical documentation.
Key Responsibilities
Write weekly and monthly clinical notes
Communicate with patients' families regarding care and treatment
Participate in multidisciplinary treatment planning meetings
Perform case management duties as a core member of the care team
Required Skills & Qualifications
Bachelor's or Master's Degree in Social Work (BSW or MSW)
Minimum Licensed Social Worker (LSW) certification required
Preferred Skills
Case management experience
Experience in a clinical or healthcare multidisciplinary team setting

Onsite - Trenton, NJ 08628
Monday-Friday, 8:00 AM - 4:00 PM (1-hour unpaid lunch)
