Summary of Duties: Using own automobile, assists Hospice patients/families to meet their psychosocial needs through assessment and appropriate intervention as needed. Acts in a consultant role to multidisciplinary staff members in Hospice. Furthers the knowledge and understanding of the social work role of Hospice through appropriate educational outreach.
Essential Functions of the Job
Evaluates the psychosocial needs of referred patients/families, taking into consideration: developmental history, current family system, need for services or community referrals.
Provides assistance with bereavement coping.
Provides social casework and counseling as needed.
Participates in the development of the Patient/Family Interdisciplinary Plan of Care, with particular attention to emotional, social, and family needs.
Provides the social work perspective at Interdisciplinary Group conferences. In cooperation with other disciplines, develops and maintains a plan of care which meets the needs of the patient/family.
Assists with Hospice Bereavement Services with a complete follow-up.
Ensures that all patient information is held in strict confidence.
Performs related administrative functions and other related duties as assigned.
Requirements
Educational Requirements: Master's in Social Work. Current licensure as a Social Worker in the State of Hawaii.
Experience: One year's previous experience. Hospice - Death & Dying experience, preferred
Skills Required: Current Hawaii driver's license with clean Driver's Abstract. Excellent oral and written communication skills. Ability to both organize and prioritize work. Strong interpersonal relationship skillsDocumented computer skills.
