Full-Time Hospice / Home Health Social Worker
Full-time Hospice / Home Health Social Worker in Spokane, WA who provides emotional, social, and practical support to patients and families while coordinating care and connecting them to community resources. The role is a key member of the interdisciplinary team, ensuring holistic, patient-centered care throughout the healthcare journey.
Key Responsibilities:
Conduct psychosocial assessments for hospice and home health patients
Provide emotional support, counseling, and crisis intervention for patients and families
Assist patients and caregivers with advance care planning and end-of-life support
Connect patients with community resources and support services such as Medicaid application assistance, Meals on Wheels, medical transportation, caregiver support, utility assistance, and housing assistance
Coordinate respite care services and hospice transfers as needed
Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
Advocate for patient rights, dignity, and quality of life
Maintain accurate documentation in compliance with hospice and home health regulations
Qualifications:
Bachelor's or Master's degree in Social Work from an accredited program
Active Idaho Social Work license
Licensed Master Social Worker (LMSW) required
Knowledge of community resources, Medicaid, and social service systems
Strong communication, organization, and time-management skills
Valid driver's license with the ability to travel to patient homes
Preferred Skills:
Experience in hospice, home health, medical social work, or healthcare setting
