The MUSC Health inpatient Social Worker assesses psychosocial needs and applies interventions in order to support the patient's medical treatment plan. The social worker will utilize interventions to enhance patient and family/caregiver functioning and emotional well-being, and access appropriate and agreed-upon services. The inpatient Social Worker is responsible for collaborating and coordinating care to enhance the patient experience and the achievement of collaborative goals. They collaboratively develop patient goals, monitor progress, address barriers to goal achievement, and evaluate outcomes of individualized care plans. This role provides training, guidance, and mentors staff and physicians on the management of complex psychosocial patient issues, in line with MUSC Health Hospital's academic mission.

$2,500 sign-on bonus available!
Education: Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
Licensure: Current and active MSW licensure issued by the South Carolina Board of Social Work Examiners is required at the time of hire.
Preferred Qualifications: Certification in Case Management is preferred
The Social Worker (MSW) I reports to the Case Management/Social Work Supervisor. Under guidance of supervisor, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions. Interviews patients and relatives to obtain social history relevant to medical problems and planning. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies. Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.
