Job description
LifeBridge Health seeks a Social Worker for our inpatient psychiatric unit to deliver compassionate, patient-centered care. In this role, you will conduct psychosocial assessments, develop treatment and discharge plans, and coordinate community resources to ensure safe, effective transitions of care. You’ll collaborate closely with psychiatrists, nurses, and a multidisciplinary team, provide supportive counseling, and participate in crisis intervention and safety planning.
