Location: Northampton, MA
Setting: Home Health
Pay Range: -
Schedule: 07:00:00 - 15:00:00 (Days)
Contract Length: 13 Weeks
Candidate Type: Travel
Requirements: 2 years of experience as a Social Worker, Qualified applicants MUST have at least 2 years of experience in the LCSW, Valid Social Worker license, Be willing to obtain Massachusetts licensure
Responsibilities
Deliver specialized care to patients in a home health setting.
Collaborate with multidisciplinary healthcare teams to coordinate patient care.
Monitor patient conditions, provide social work interventions, and document outcomes.
Advocate for patient needs to support recovery, safety, and continuity of care.
Assist with discharge planning and complex care coordination.
