Medical Social Worker LCSW Home Health | Full-Time | Eugene, OR
Make a Meaningful Impact in Hospice Care
Signature Eugene Home Health is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This role is ideal for a social worker passionate about psychosocial support, and patient advocacy .
As a Home Health Social Worker , you will provide critical emotional support, care coordination, and community resource guidance to patients and families navigating life's most meaningful transitions.
What You'll Do (Key Responsibilities)
Conduct comprehensive psychosocial assessments for patients and families
Develop and implement care plans based on social and emotional needs
Provide grief counseling, bereavement support, and crisis intervention
Coordinate community resources, referrals, and support services
Serve as a liaison between patients, families, and healthcare providers
Collaborate with the interdisciplinary home health care team (RN, physician, aides)
Participate in quality assurance and performance improvement initiatives
What You Bring
Licensed in Social Work (LCSW) from an accredited program
1+ year of healthcare experience (hospice or home health preferred)
Strong communication, organization, and interpersonal skills
Ability to work independently in a home health / field-based setting
Current CPR certification
Valid driver's license and reliable transportation
