Social Services - MSW (Masters of Social Work)
Schedule
Day shift, 5x8 hours, from 08:00 to 16:30.
Assignment Details
Typical weekend requirement: one full weekend per 4 weeks, subject to department needs. No block scheduling is offered.
Requirements
2 years of acute medical Social Work experience, including roles in Social Work and discharge planning.
First-time travelers must pass a screening process.
No time-off restrictions greater than 7 days.
Responsibilities Provide comprehensive assessments and interventions for patients and families.
Facilitate discharge planning and coordinate services for continuity of care.
Advocate for patients' needs and assist in navigating healthcare systems.
Collaborate with healthcare teams to develop discharge plans.
Maintain accurate documentation of services provided.
