What You'll Do
\ NAs a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:
\ N Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
\ N Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
\ N Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
\ N Guiding families through Advanced Directives with clarity and sensitivity
\ N Assisting with facilityplacement for patients when appropriate
\ N Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
\ N Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
\ N Participating in on-call duties on evenings and weekends as needed
\ N Managing caseload whileAssuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

\ NWhat You Bring
\ N Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
\ N Licensure and/or certification as required by the State.
\ N Minimum of 2+ years social service experience in a health care setting or social services agency.
\ N Experience in hospice, palliative care, or a related healthcare setting preferred
\ N Deep comfort with grief, loss, and end-of-life conversations
\ N Strong knowledge of community resources and care coordination
\ N Excellent documentationskills and attention to compliance
\ N Self-directed with the ability to work with little supervision
\ N Valid driver's license and willingness to travel locally for patient visits
