Social Worker Position at Blakeford at Green Hills
Blakeford at Green Hills, a non-profit continuing care retirement community nestled on ten acres in beautiful Green Hills, is actively seeking an experienced Social Worker to guide the resident experience in a long-term care environment. As a Blakeford Social Worker, you will counsel residents and their families on emotional, environmental or physical observations and interventions. Conduct regular face-to-face visits with assigned residents, facilitate active participation in care planning, serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities, and compile performance metrics and present summary data.
