Schedule: PRN as needed
Salary Range: $31.00 - $38.00 an hour

What You'll Do
As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:
* Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
* Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
* Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
* Guiding families through Advanced Directives with clarity and sensitivity
* Assisting with facility placement for patients when appropriate
* Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
* Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
* Participating in on-call duties on evenings and weekends as needed
* Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

What You Bring
* Master's degree from school accredited by the Council on Social Work Education required in states as specified.
* Licensure and/or certification as required by the State.
* Minimum of 2+ years social service experience in a health care setting or social services agency.
* Experience in hospice, palliative care, or a related healthcare setting preferred
* Deep comfort with grief, loss, and end-of-life conversations
* Strong knowledge of community resources and care coordination
* Excellent documentation skills and attention to compliance
* Self-directed with the ability to work with little supervision
* Valid driver's license and willingness to travel locally for patient visits
