Job Summary:
Department : Social Service
This job is for 8-hour shifts and five days each week, with the shift schedule being from 08:00 AM to 16:30 PM, a total of 40 hours per week.
We are looking for a Social Worker with 2 Professional References.
Working Days: Wed, Thurs, Fri, Sat, Sun
Social work license.
MSW
Discharge Planning/Crisis Intervention/Monitor Length of stay for Goals Birth & Women's Unit/ED/Medical/Surgical/ICU Rotating throughout facility.
Roles & Responsibilities: Implement and monitor patient care plans.
Monitor, record and communicate patient conditions as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
ssess and coordinate patient's discharge planning needs with members of the healthcare team.
