Health Care For Homeless Veterans Hud-Vash Intake Coordinator
As the Health Care for Homeless Veterans (HCHV) & HUD-VASH Intake Coordinator, functions as part of the interdisciplinary team and provides psychosocial services primarily to Veterans who are experiencing, or at risk of homelessness and in need of specialized support. Provides a combination of direct patient care services to Veterans being served by the Homeless Patient Aligned Care Team (H-PACT), as well as clinical and administrative service coordination for Veterans accessing HCHV programs.
