Job description
Social Worker
We are seeking a compassionate and experienced Social Worker to join our long-term care team at our skilled nursing facility in Providence, RI. As a Social Worker at AdviniaCare Providence, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Our facility is committed to providing high-quality services that prioritize the unique needs and well-being of each individual.
Responsibilities:
Assists residents and their families in connecting with community resources, healthcare providers, and other social services
Conducts assessments and evaluations to identify psychological, financial, religious, and physical needs
Develops and implements individualized care plans that meet the unique needs of each resident
Collaborates with other disciplines to ensure comprehensive care and support
Provides counseling and support services to residents and their families
Maintains accurate and detailed records of resident interactions and services provided
Requirements:
Education: Bachelor's of Science in Social Work or a related field
Experience: Minimum 2 years of experience as a Social Worker
License: LSW, LCSW or LICSW preferred but not required
Skills: Strong communication, assessment, and social work methods and techniques; ability to work effectively with a variety of disciplines; knowledge of social systems and ability to implement pertinent state and federal regulations
